I did notice that your aggression settings were 60 and I always caution that what works for my mount doesn't necessarily mean it will work for someone else, but it gives you another avenue to try if need be.
What I remember from using PHD2 many moons ago was whatever they recommended I followed within reason. I would really play around with those aggression settings and see if it improves. Maybe that mm guide scope is too long. I started with an 80mm Orion Short Tube scope and an orion star-shooter guide camera. I finally ended up with an off-axis guider, using my main camera as my guide camera and got much better guiding results. Any views in here are independent.
